Home Visit Requests

At our practice, we understand that sometimes patients can’t attend the medical centre due to illness, frailty, or mobility issues. In these cases, you may request a doctor’s home visit.

Please note: Home visits are not guaranteed and are subject to doctor availability and clinical assessment.

How to Request a Home Visit:

1. Call the clinic on (07) 3806 1544 as early as possible (preferably before 10:00am).
2. Provide the receptionist with:
– Patient’s full name
– Address and contact number
– Reason for home visit
3. The doctor will review your request and confirm if a visit can be arranged.

Important Notes:

• Home visits are only available within the local area.
• Urgent medical emergencies should call 000 immediately.
• Some services may not be available during home visits.
